If you want the calls to forward to multiple numbers, you will have the ability to manage this after your account has been approved and provisioned. For the purpose of signing up, just list the first number to which calls should forward. If forwarding to a SIP address or VoIP account, just specify VOIP/SIP (which is the default setting in Step 2 on the signup form). - Click on View Rates & Try for Free
You'll then see the five (5) tiers of monthly pre-paid plans. After selecting your desired plan, you'll have the option to add Outbound Calling and additional features as you go through the signup process.
